This work arises from a hepatitis C cohort study and focuses on estimating the effects
of covariates on progression to cirrhosis. In hepatitis C cohort studies, patients may
be recruited to the cohort with referral bias because clinically the patients with more
rapid disease progression are preferentially referred to liver clinics. This referral bias
can lead to significantly biased estimates of the effects of covariates on progression
to cirrhosis.
